Medical Form

All New City Little League players must have a medical form filled out for the current season.  The information must be carried by the team manager at all games and practices in the event of injury.  Starting in 2024, the form will be auto filled based on the information provided on the player registration form. 

In the event that information changes, you may access this form here.  This form will also need to be filled out by players selected to play All Stars.
